  3. Community Health/Chronic Disease Mgmt. Project:   Required: 20% of final grade
    1. Review CDM primer in the project tools folder and the "Improving Chronic Illness Care" CD
    2. Submission of the powerpoint template with your information {template in CDM folder: CDM template.ppt
      1. Template must be downloaded and saved on your computer or your Clerkship disk
      2. Lecture notes mode provides guidance for project presentation
      3. Focus on the community resources and how they apply to your patient and the practice population
      4. This is NOT a case presentation
      5. DO NOT FORGET PREVENTION in your overall plan for management

    3. Submission of your disease registry without patient identification. (registries folder) Submission of material with personal health information will result in a "0" evaluation on this portion of the course.
      1. select 2 diseases or 1 diseases and 1 preventive measures.
      2. fill in every patient you see with the designated problem until you meet the required number for the target (see introduction sheet of each registry)
      3. delete the patient identification at the end of the rotation
      4. write a 1-2 page paper about what you learned re: disease management and the use of a disease registry
        1. See intro page of registries for sample discussion questions
        2. 1 paper, not 4.  You are discussing process, not individual diseases
    4. We will discuss Chronic Disease Management at debriefing.
    5. Turn in presentation or write-up electronically 24 hours prior to debriefing (send to Jody Crabtree @ crabt003@mc.duke.edu).  You may use the template provided and clarify your information using the speaker notes.
    6. Discuss the community health project with your preceptor.  Start early.
  4. SOAP notes: Required:  15% of final grade
    1. Prepare two SOAP notes for faculty review. 
    2. The notes must be on two different common problems and two different patients. 
    3. Submit the notes electronically on the evening before debriefing
    4. Templates that outline required data available for many of the problems at the common problems website at the bottom of the learning objectives of each major problem
    5. Include a problem list and medication list at the beginning
    6. Evaluation based on:
      1. Organization
      2. Content
      3. Evidence of thoughtful assessment based on subjective and objective content
      4. Plans consistent with assessment
        1. Guideline driven when applicable
        2. Evidence based
        3. Complete

You MUST put your name on the note and you MUST NOT include the patient's name or initials.

Failure to follow this rule will result in a 0 on this component.

  1. Real World CD: Required
    1. Review and complete modules. 
    2. Evaluation will be included on the final exam.
    3. You do NOT have to turn in the quiz
  2. INTERIM EVALUATION
    1. Review with your preceptor midway the course.
    2. Turn in a copy of the midterm evaluation at debriefing
  3. Performance evaluation by Preceptor: 50% of final grade
    1. There is a copy in your folder and on your CD

    2. Due at debriefing

    3. The final grade cannot be completed without the form

  4. Course evaluation: Required
    1. Complete the course evaluation. 
    2. Department forms provided at debriefing
    3. School of Medicine forms on BlueDocs.
    4. Grades will NOT BE SUBMITTED without acknowledgement of form completion and student log submission.
  5. Final exam:  Required: 15% of final grade
    1. 8:00 a.m. sharp on debriefing day
    2. One hour allowed for completion
    3. 50 multiple choice questions based on goals and objectives for common problems and required modules. (Chart audit, Quality improvement, Real World CD, Lectures).
  6. Skills assessment form (pre- and post- ) should be submitted electronically at debriefing.
  7. BlueRecs Clinical Log.  We will be reviewing these during the course of the rotation. (REQUIRED)
