CHRONIC DISEASE MANAGEMENT: CASE PRESENTATION
Chronic Disease Management Case Presentation

The  CDM case report guides the student through the components of CDM with a focus on a patient that they meet in the practice.  

The template for the case report is available online. The grading criteria are also available.

 

Encourage your student to review all of the patients who they encounter with chronic disease for surveillance parameters.  Let them help develop a prospective care profile.  It should be a win-win-win situation for you, the patient and the student.

 

The student will be required to select a patient who  has a chronic disease for this project.  The object of the exercise is to have the student do a comprehensive assessment of the guidelines, skills, tools, and resources necessary for ongoing patient-centered care.  This will include:

bulletA patient profile
bulletA review of preventive service needs
bulletA review of ongoing management of the chronic disease
bulletIdentification of barriers and attributes that impact care
bulletHelping the patient set an achievable short term goal
bulletIdentifying and assessing local resources including practice resources for assisting the patient
bulletIdentifying medical management goals and surveillance needs
bulletIntroducing the patient to the resources

In addition, each student will maintain a minimum of five care logs or "registries" addressing chronic disease surveillance.

Skills Assessment Your student should present you with a self assessment of skills on day 1.  This is the student's evaluation of their current level of knowledge and skills based on the common problems. 

Use this as a guide for the student's learning plan.  It is ultimately the students responsibility to review this.

Interim feedback  The student is required to seek a mid-rotation review of progress.  You will receive a form from the student to accomplish this task.  You should also receive a list of the problems the student has seen during the interim. Interim assessment
Final Assessment The final grade for the student consists of: your evaluation (50%), the CDM case and registries (20%), SOAP notes (15%) and an exam (15%).
